God created 2 sexes, male and female. God does NOT make mistakes. If people are not sure what they are then yes they need help but not from a surgeon, but mental and spiritual help.
As far as sports go, a simple DNA test will show if someone is XX or XY. XX only in women's sports XY in men's sports. Seems really pretty simple. But sadly to many will claim something to be able to cheat and others will bend over backwards to help them cheat.
