Three applicants have filed certificate of need applications with the North Carolina Department of Health and Human Services to develop a Medicare-certified Home Health Agency

    GREENVILLE, N.C.     Three applicants have filed certificate of need applications with the North Carolina Department of Health and Human Services to develop a Medicare-certified Home Health Agency in response to a need determination in the 2023 State Medical Facilities Plan for one Medicare-certified Home Health Agency in Pitt County.

    Each applicant proposes one project:

    Five Points Healthcare of NC, LLC proposes to:

  • Develop a Medicare-certified Home Health Agency -- Aveanna Home Health-Pitt - pursuant to the need determination in the 2023 State Medical Facilities Plan. The project is expected to cost $125,464 and would be completed in July 2024.

    BAYADA Home Health Care, Inc. proposes to:

  • Develop a Medicare-certified Home Health Agency -- BAYADA Home Health Care, Inc. - pursuant to the need determination in the 2023 State Medical Facilities Plan. The project is expected to cost $115,013 and would be completed in April 2025.

    Well Care Home Health of Pitt, Inc. proposes to:

  • Develop a Medicare-certified Home Health Agency -- Well Care Home Health of Pitt - pursuant to the need determination in the 2023 State Medical Facilities Plan. The project is expected to cost $100,000 and would be completed in January 2025.

    A public hearing for these projects will be held Dec. 15 at 10:30 a.m. in the Eugene James Auditorium on the second floor of the Pitt County Administrative Office Building, 1717 W. 5th St., in Greenville.

    Anyone may file written comments concerning this proposal. Comments must be received by the Healthcare Planning and Certificate of Need Section no later than 5 p.m. on Dec. 1.

    Comments may be submitted as an attachment to an email if they are sent to DHSR.CON.Comments@dhhs.nc.gov. Comments may also be mailed to the following address:
